Cleaner implementation of MBEDTLS_IGNORE_RETURN

The previous implementation was misparsed in constructs like
`if (condition) MBEDTLS_IGNORE_RETURN(...); else ...;`.

Implement it as an expression, tested with GCC, Clang and MSVC.
