Improve CCM documentation

- Rephrase function/parameter/enum/define/error descriptions into full and
  clear sentences.
- Make sure to adhering to the Arm writing guidelines.
- Fix missing/incorrect Doxygen tags.
- Standardize terminology used within the file.
- Fix iv_len values per the standard.
