Several files among include/psa/crypto_*.h are not meant to be included
directly, and are not guaranteed to be valid if included directly. This
makes it harder to perform some static analyses. So make these files more
self-contained so that at least, if included on their own, there is no
missing macro or type definition (excluding the deliberate use of forward
declarations of structs and unions).

Move fields around to have fewer accesses outside the 128-element Thumb
direct access window.
In psa_hkdf_key_derivation_t, move the large fields (output_block, prk,
hmac) after the state bit-fields. Experimentally, it's slightly better
to put hmac last.
In aead_operation_t, tag_length was outside the window. The details depend
on the sizes of contexts included in ctx. Make the large ctx be the last
field.
In mbedtls_psa_hmac_operation_t, the opad field is outside the window when
SHA-512 is enabled. Moving opad before hash_ctx only saves 4 bytes and made
the structure clumsy, so I left it alone.
Results (arm-none-eabi-gcc 7.3.1, build_arm_none_eabi_gcc_m0plus build):
library/psa_crypto.o: 16246 -> 16166 (diff: 80)

According to the PSA specification the PSA_USAGE_SIGN_HASH has the
permission to sign a message as PSA_USAGE_SIGN_MESSAGE. Similarly the
PSA_USAGE_VERIFY_HASH has the permission to verify a message as
PSA_USAGE_VERIFY_MESSAGE. These permission will also be present when
the application queries the usage flags of the key.

It makes sense to do the length checking in the core rather than expect
each driver to deal with it themselves. This puts the onus on the core to
dictate which algorithm/key combinations are valid before calling a driver.
Additionally, this commit also updates the psa_mac_sign_finish function
to better deal with output buffer sanitation, as per the review comments
on #4247.

For cipher multi-part operations, dispatch based on
the driver identifier even in the case of the
Mbed TLS software implementation (viewed as a driver).
Also use the driver identifier to check that an
cipher operation context is active or not.
This aligns the way hash and cipher multi-part
operations are dispatched.

Drivers (both built-in and external) need to declare their context
structures in a way such that they are accessible by the
to-be-autogenerated crypto_driver_contexts.h file. That file lives in
include/psa, which means all builtin driver context structure
declarations also need to live in include/psa.

Apparently there's a goal to make the PSA Crypto core free from
dynamic memory allocations. Therefore, all driver context structures
need to be known at compile time in order for the core to know their
final size.
This change defines & implements for hashing operations how the context
structures get defined.

The lifetime of key attributes now encodes whether a key is
volatile/persistent or not AND its location.
Fix PSA code where the fact that the lifetime encodes
the key location was not taken into account properly.
Fix the impacted tests and add two non regression tests.

Once an operation has been 'accepted' by a driver, the remainder is bound
to the same driver, since driver-specific context structs cannot be shared.
This provides a pretty good gate mechanism for the fallback logic, too.

With PSA crypto v1.0.0, a volatile key identifier may
contain a owner identifier but no file is associated
to it. Thus rename the type psa_key_file_id_t to
mbedtls_svc_key_id_t to avoid a direct link with a
file when a key identifier involves an owner
identifier.
The new type name is prefixed by mbedtls to highlight
that the type is specific to Mbed TLS implementation
and not defined in the PSA Cryptography API
specification.
The svc in the type name stands for service as this
is the key identifier type from the point of view of
the service providing the Cryptography services.
The service can be completely provided by the present
library or partially in case of a multi-client service.
As a consequence rename as well:

The purpose of this commit and the following is for
psa_key_id_t to always be as defined by the PSA
Cryptography API specification.
Currently psa_key_id_t departs from its specification
definition when MBEDTLS_PSA_CRYPTO_KEY_FILE_ID_ENCODES_OWNER
configuration flag is set. In that configuration, it is set
to be equal to psa_key_file_id_t which in that configuration
encodes an owner identifier along the key identifier.
Type psa_key_file_id_t was meant to be the key identifier type
used throughout the library code. If
MBEDTLS_PSA_CRYPTO_KEY_FILE_ID_ENCODES_OWNER is set it
includes both a key and owner identifier, otherwise it is
equal to psa_key_id_t.
It has not been the key identifier type throughout the
library so far because when the PSA Cryptography
specification was developped the library Doxygen
documentation was used to generate the PSA Cryptography API
specification thus the need to use psa_key_id_t and not
psa_key_file_id_t.
As this constraint does not hold anymore, move
to psa_key_file_id_t as the key identifier type throughout
the library code.
By the way, this commit updates the key identifier
initialization in the tests to be compatible with a
composit key identifier. A psa_key_id_make()
inline function is introduced to initialize key
identifiers (composit ot not) at runtime.

All key types now have an encoding on 32 bits where the bottom 16 bits
are zero. Change to using 16 bits only.
Keep 32 bits for key types in storage, but move the significant
half-word from the top to the bottom.
Likewise, change EC curve and DH group families from 32 bits out of
which the top 8 and bottom 16 bits are zero, to 8 bits only.
Reorder psa_core_key_attributes_t to avoid padding.
If none of the inputs to a key derivation is a
PSA_KEY_DERIVATION_INPUT_SECRET passed with
psa_key_derivation_input_key(), forbid
psa_key_derivation_output_key(). It usually doesn't make sense to
derive a key object if the secret isn't itself a proper key.
A macro useful for initializing psa_key_id_t, whether
MBEDTLS_PSA_CRYPTO_KEY_FILE_ID_ENCODES_OWNER is set or not. Without this
macro, it is necessary to know if
MBEDTLS_PSA_CRYPTO_KEY_FILE_ID_ENCODES_OWNER as with it the key ID is
non-scalar and needs to be initialized with {0, 0}, and 0 otherwise when
key ID is scalar.
Avoid an error with differing linkages being expressed for
psa_set_key_domain_parameters() between crypto_extra.h and
crypto_struct.h in C++ builds.

Add a slot_number field to psa_key_attributes_t and getter/setter
functions. Since slot numbers can have the value 0, indicate the
presence of the field via a separate flag.
In psa_get_key_attributes(), report the slot number if the key is in a
secure element.
When creating a key, for now, applications cannot choose a slot
number. A subsequent commit will add this capability in the secure
element HAL.
